Authorization for Minor Consent
Kindly note, patients under the age of 18 are considered minors and cannot be seen without a parent or legal guardian present unless a signed Authorization for Minor Consent form has been submitted.
Online Payment Portal
You can now view your statement and pay your bill by logging in to your patient portal.

Patient Portal
At your first appointment you should be offered a username and password to access your Patient Portal. This will allow you to view online your medical records from our office.
To Log into your new patient portal please go to the URL: Gamtnderm.ema.md
Then use the username and password given to you by the receptionist at your last check-in.
You'll then be able to change your password and view your medical records for visits to our office.
